Wedding Ceremony Request Form

Congratulations on your upcoming wedding! As a church family, we are excited to embark on this journey with you to make your day extra special.

Please complete this form with enough detail to enhance our planning.

A few things to note:

We require a minimum of 4 months to plan, however, we may be able to accommodate requests within this timeframe based on case-by-case approval from our Lead Pastor.
Your wedding is only confirmed after approval from our Events Coordinator.
If you would like us to be involved in your wedding reception in some capacity, please state this in the 'special request' section at the bottom of the page.

All requests will be taken into consideration and a final decision will be communicated to you within 3-5 days.
